At what point during titration would a clinician usually decide a dose has plateaued?

So normally this would be when their is no further meaningful benifit to increasing the dose, and that the side effect profile is begininning to get worse VS the benifit being recived. so lets say you are on 40mg lisdex, and we go to 50mg lisdex, you notice only a slight improvement but your side effects of dry mouth, insomnia and headache begin to emerge. that would be a plateu.
